a. collect, use, disclose and/or process personal data about me that I had provided 7 Vision Pte Ltd, that I now provide 7 Vision Pte Ltd, that I may in future provide 7 Vision Pte Ltd with and/or that 7 Vision Pte Ltd possesses about me including but not limited to my name, my date of birth, my identification number, my email address, my telephone numbers, my address for the following purposes:
• Providing medical services to me and meeting my healthcare needs within 7 Vision Pte Ltd.
• To avail the services which the Group provides, including, where necessary, the transferring to or sharing of my personal data with third party medical service providers such as specialists, imaging providers, pathology providers, general practitioners or other allied health professionals such as physiotherapists.
• Use my data to generate an aggregate anonymised corporate wellness report to share with my employer
• Where there is a serious and imminent threat to an individual’s life, health, or safety.
• Where there is a serious threat to public health or public safety.
• Resolving complaints and dealing with enquiries made by me.
• Maintenance and updating of the data.
• Administrative or operational purposes.
• Processing credit notes and processing refunds.
• Collection of fees, charges and expenses for services provided.
• Verification and identification purposes;
• Filing of medical claims on my behalf with the relevant company, employer or insurance provider.
• Collecting payments by credit card, cheque, bank transfers or other means.
• Carrying out billing, accounting, auditing and the maintenance of proper book-keeping for the Practice’s operations and business.
• The disclosure of the relevant books, documents, records and information (in hard or soft copy) to the auditors for the preparation of financial reports.
• Compliance with the applicable laws and regulations.
• facilitating, processing, dealing with, administering, managing and/or maintaining our relationship with 7 Vision Pte Ltd;
• dealing in any matters relating to 7 Vision Pte Ltd products, services and activities;
• carrying out due diligence and other screening activities, or to prevent or investigate any fraud or breach of contract or unlawful activity, or complying with or as required by any applicable law, governmental or regulatory requirements of any relevant jurisdiction;
• conducting research, analysis and development activities (including but not limited to data analytics, surveys and/or profiling) to enhance 7 Vision Pte Ltd products, services and/or operations, and/or otherwise for 7 Vision Pte Ltd business operations and purposes;
• storing, hosting, backing up (whether for disaster recovery or otherwise) of my personal data, whether within or outside Singapore,
(collectively the “Purposes”);
b. disclose personal data about me to 7 Vision Pte Ltd third party service providers and agents (“Representatives”) that are engaged by 7 Vision Pte Ltd to perform certain functions in connection with the Purposes, so long as disclosure is necessary to enable such Representatives to perform the said functions. The Representatives may be located within or outside Singapore. In this regard, I hereby acknowledge, agree and consent that the said Representatives may, to the extent necessary to perform the said functions, collect, use, disclose and/or process my personal data.
Data Retention and Rights of Access/Correction
7 Vision Pte. Ltd. will retain my personal data in accordance with its data retention policy, relevant MOH guidelines, and applicable laws, and only for as long as is necessary to fulfill the Purposes or as required by law. I also understand that I may request access to, and correction of, my personal data by contacting 7 Vision Pte. Ltd.’s Data Protection Officer at [email protected].
Should you wish to withdraw your consent in part or whole, please send an email to our Data Protection Officer at [email protected] and provide details of your withdrawal.
